The Referral Process
The individual must be registered with the county Administrative Entity and Office of Developmental Programs. A referral packet must be submitted to CSS by the individuals Supports Coordinator. This packet will include the individual’s current physical, most recent psychological, ISP and a Social Summary compiled by the individual’s SC. Once this packet is received and reviewed by our intake Coordinator, an acceptance letter will be sent and a team meeting will be scheduled to discuss the level of care the individual needs.
The ISP will be updated to reflect the needs of that individual. After the service is authorized by Intellectual Disabilities Services/AAA, a staff will be assigned and services will be rendered. The family and the individual are encouraged to meet and talk with the staff before the service begins, to see if it’s a good match.
